This webinar highlights an online platform that enables patients to meet in virtual small groups and receive coaching on self-management skills and lifestyle changes.

About 54 million Americans have prediabetes, when blood glucose levels are above normal but do not qualify as diabetes. Such patients are at risk of developing type 2 diabetes and/or heart disease unless they lose weight and become more active.

To address prediabetes, Omada Health created Prevent, an online program based on guidelines from the National Diabetes Prevention Program. The program is guided by dedicated health coaches and offers community social support, an evidence-based curriculum, and digital tracking tools that include a wireless scale and mobile app.

The program has been adopted by several employers and health plans. Outcomes published in The Diabetes Educator and the Journal of Medical Internet Research show that Prevent helps patients lose weight and make lifestyle changes that reduce the risk of developing type 2 diabetes.
