About this grant

To improve access to behavioral health services, strengthen the degree of physical health and behavioral health integration, and improve outcomes for patients at 15 community health centers in California, as well as to identify the key capabilities required to deliver integrated behavioral health care to individual patients and priority subpopulations in safety-net primary care settings, with an emphasis on equity and attention to social needs.
