About This Event

CareOregon, a nonprofit health plan serving Medicaid and Medicare recipients in Oregon, pairs specially trained, nonmedical community outreach workers with patients who are frequent hospital and emergency department services users.

Working with physicians and multidisciplinary care teams in the primary medical home and relevant specialty practices, the community outreach workers provide individualized, high-touch support to patients, both in community sites and in patients’ homes. Support services include care coordination, advocacy, health education, coaching, and assistance with life skills.
