Transitions Clinic Network (TCN) is a national network of medical homes for individuals with chronic diseases recently released from incarceration. Because it is founded on the idea that the people closest to the problem are also closest to the solution, each clinic that adopts the Transitions Clinic Program employs a community health worker with a history of incarceration as part of the clinical team.

The California Health Care Foundation is funding TCN to support up to 25 county and community health centers throughout California in implementing the TCN program.

In this webinar, Shira Shavit, MD, executive director of the Transitions Clinic Network, described the core components of the TCN program and provided details about the opportunity to become an expansion site. Proposals are due by November 16, 2018.
