About this grant

To advance the understanding of essential care delivery functions and aligned financial and health information technology capabilities that safety-net practices must have in order to provide high-quality, comprehensive primary care in a value-oriented payment environment and to illustrate approaches for how these capabilities can be built by and shared among various clinics and community health centers.
