The Center for Medicare & Medicaid Innovation (the Innovation Center) was established at the Centers for Medicare & Medicaid Services (CMS) through the Affordable Care Act (ACA) to test innovative payment and care delivery models to reduce costs and maintain or improve quality of care for recipients of Medicare, Medicaid, and the Children's Health Insurance Program. The Bundled Payments for Care Improvement Initiative includes four models that link payments for multiple services that beneficiaries receive during an episode of care. A bundled payment is a single amount for services related to a clinical condition in a specified care episode. In Model 1 the episode of care is defined as the inpatient stay in the acute care hospital. Model 2 focuses on inpatient stay plus post-acute care. Model 3 focuses on post-acute care only. Model 4 includes a prospectively determined bundled payment to hospitals for all services provided during an inpatient stay.

The FQHC Advanced Primary Care Practice Demonstration examines how the patient-centered medical home model can improve quality of care, promote better health, and lower costs at Federally Qualified Health Centers. Participating FQHCs are expected to help patients manage chronic conditions, and actively coordinate care for patients. To assist them in doing so, participating organizations are paid a monthly care management fee for each eligible Medicare beneficiary receiving primary care services.

The Strong Start for Mothers and Newborns Initiative includes two major strategies: promoting awareness and spreading best practices to reduce early elective deliveries through the hospital networks of the Partnership for Patients initiative; and testing three evidence-based, enhanced prenatal care interventions for women enrolled in Medicaid or CHIP who are at risk for having a preterm birth.
