Chronically ill patients and the clinics that serve them may benefit from the use of chronic disease management systems (CDMSs), which can track and present data for thousands of patients at a time and automate care follow-up processes. A CDMS can enable providers to scale up their quality improvement protocols to a level that is unattainable through traditional chart audits.
This 2008 issue brief profiles four health centers in California that were using various CDMSs and related tools to efficiently care for patients with asthma, diabetes, hypertension, and other chronic conditions. The populations served by these health centers were low-income, underserved, and most vulnerable to disparities in treatment for chronic conditions. The clinics in the issue brief are:
