CHCF is proud to support eight visionary organizations that are working to create a safer and stronger health system for Black Californians.
In late 2023, CHCF put out a call for projects that would improve clinical quality of care for Black Californians. The response was voluminous, with community clinics, hospitals, and technology start-ups from across the state proposing interventions that addressed the care continuum.
These organizations were selected for their deep knowledge of the interaction among health systems, cultural strengths, and health outcomes. Their projects address health needs from culturally competent prenatal care to hypertension management to colorectal cancer screening.
From 2024 to 2026, the Pulse of Change leaders will conduct their interventions, assess impact toward their stated goals, and, if appropriate, implement a sustainability plan to integrate their interventions into clinical practice.
