Community health workers, promotores, and community health representatives (CHW/P/Rs) can play a valuable role in collecting data, transferring information between health and social service systems, and informing MCPs, FQHCs, hospital systems, and the state about community needs. CHW/P/Rs can also provide the most up-to-date information on elements such as member housing status, contact information, and any immediate or emergent health risks. Whether CHW/P/Rs coordinate care as part of an interdisciplinary primary care team in an office-based setting or engage community members in the field, they are well positioned to collect this type of information.

CHCF has gathered a variety of resources related to data collection and program management:
