National and statewide initiatives — such as the movement of dual eligibles and Medicaid recipients into managed care, and the inclusion of long-term services as a Medicaid managed care benefit — aim to increase care coordination and place greater responsibility on participating managed care plans to provide long-term services in the most cost-effective setting. Because the transition of these populations and services is relatively new, many participating health plans and medical providers may be unfamiliar with the role that various housing options can play in the provision of long-term services.

This report provides an overview of the supportive housing options available in California to older adults and individuals with disabilities. The settings discussed include:

• Licensed facilities such as adult residential facilities, skilled nursing facilities, and residential care facilities for the elderly
• Unlicensed housing options such as publicly subsidized rental properties and permanent supportive housing

In addition, this report summarizes ways managed care plans in other states have developed relationships with different housing partners to provide long-term services, and offers suggestions for how California plans might do the same.
