CIN Webinar — Developing a Learning Community to Advance Care Management

About This Event

Community Health Center Network (CHCN), a CIN partner, has successfully completed its first pilot cohort of a peer-learning and professional development program for care management and care coordinator professionals, such as community health workers. The program, the Care Management Learning Community (CMLC), started in June 2020 as a result of funding from a CIN capacity-building grant, and is an educational model that helps address structural determinants of health and health equity by developing the skills of allied health professionals on health care teams. The CMLC exceeded the project’s objectives by building a vibrant learning community cohort of care managers, breaking down silos between care coordinator groups, and much more.

The CIN webinar with Jana Kantor, learning and development manager at CHCN, and Saleena Gupte, integrated care consultant and integrative medicine practitioner, shared insights on achievements and the process of program development, and what is coming next for this program.

Objectives:

  • Recognized the astounding potential of your care managers and care management teams.
  • Heard how CHCN developed and implemented a successful program to strengthen care management and care coordinator skill sets and to incorporate whole-person care within their network.
  • Learned insightful strategies for implementing successful learning opportunities and how your organization can produce a similar program.
