Palliative Care in Medi-Cal (SB 1004) Resource Center

Patient Population

Successful implementation of SB 1004 requires palliative care providers and health plan partners to identify patients who qualify for services. This section of the Resource Center includes a review of SB 1004 eligibility criteria, characteristics of the eligible patient population, and strategies for identifying eligible patients.

An overview slide deck (PDF) covers core concepts, including SB 1004 eligibility criteria and guidance on possible information sources to assess eligibility; characteristics of the eligible patient population, including typical use of health care services in the absence of SB 1004 palliative care; and findings from an SB 1004 PC pilot. Also included is a review of strategies used by health plans and providers to identify patients who are potentially eligible, and lessons from the field related to the need for palliative care generally and identifying patients who may need it.

The summary of the Partnership HealthPlan of California palliative care pilot (PDF) provides useful information about the characteristics and needs of enrolled patients, as well as a description of how delivering this care impacted costs and use of health care services.

Plans and palliative care providers can use SB 1004 Promising Practices: Identifying Eligible Patients to identify ways they may add to or modify their current approaches to identifying eligible patients.
