Palliative Care in Medi-Cal (SB 1004) Resource Center

Optimizing for Success

Health plans and palliative care providers need to appreciate the range of factors that promote delivery of quality care and an organization’s ability to sustain a palliative care service. This section of the SB 1004 Resource Center reviews factors that promote success in launching and sustaining these programs.

The overview slide deck (PDF) covers three core content areas:

  • Conditions and supports required to optimize the delivery of palliative care
  • Approaches to assessing the capacity of PC providers and identifying and addressing any gaps in readiness to deliver SB 1004 palliative care
  • Recommendations for promoting program sustainability

Plans and PC providers can use the Connecting Palliative Care Partners Worksheet (PDF) to identify all the programs and services that can be offered to patients to supplement SB 1004 palliative care.

The Gap Analysis Worksheet (PDF) can be used to evaluate palliative care provider readiness and determine how any identified gaps might be filled.

Promising Practices: Plan and Provider Collaboration (PDF) describes practices that have been identified and endorsed by plans and PC providers delivering SB 1004 PC.

Finally, two brief case studies presented at a CHCF SB 1004 technical assistance workshop in 2018 describing approaches to SB 1004 palliative care, by the Health Plan of San Joaquin (PDF) and Outreach Care Network (PDF), are also helpful.
