California’s community health centers and public hospital systems are increasingly caring for an aging and more complex patient population. In response, promising strategies and models of care tailored to this population have begun to percolate across the California safety net.

The California Health Care Foundation (CHCF) has partnered with Pacific Health Consulting Group to produce a report highlighting the key role safety-net providers can play to support the health and well-being of an older and more racially diverse population in California.

During this webinar, one of the report’s authors shared key findings, and a panel discussion explored why and how safety-net providers can expand services for older adults with low incomes by building on the providers’ experiences and strengths in delivering integrated medical care, behavioral health care, and social supports.
