The HHIP Implementation Toolkit for CoCs (PDF) contains guidance and planning documents to support Continuums of Care (CoCs) and homeless assistance partners in engaging with their local Medi-Cal managed care plans to efficiently and impactfully implement the Housing and Homelessness Incentive Program (HHIP).
The toolkit provides CoCs with critical information about the HHIP program and includes guides for CoCs to communicate with managed care plans about their own systems, goals, strengths, and limitations. Each part of the toolkit can be referenced and used separately, depending on the needs of each CoC and community partner.
The HHIP Implementation Toolkit for CoCs was produced with CHCF support to facilitate long-term cross-system coordination and collaboration to improve housing and health outcomes for people experiencing homelessness.
