Pair Team provides a turnkey case management solution, including clinical oversight and care coordination, for primary care providers and Medi-Cal health plans that do not have the resources, staffing, or expertise to launch effective complex case management programs.

The company offers a proven program for Enhanced Care Management (ECM), a new Medi-Cal benefit for people with complex needs that complements primary care and helps improve patient outcomes. ECM is a key part of CalAIM — California’s initiative to revamp Medi-Cal — that seeks to address the clinical and nonclinical needs of enrollees, particularly those with complex care needs, through the coordination of health care and social services.

Pair Team employs community health workers (CHWs) and an interdisciplinary clinical care team that includes a nurse practitioner, social worker, registered nurse, and case manager. As a virtual first model, Pair Team’s CHW engagement has been through phone calls or texts initially, although CHWs are based locally, with lived experience and knowledge about local resources.

The company was founded by Neil Batlivala, CEO, and Casse Choi, RN and COO. They launched Pair Team in 2020 with two Federally Qualified Health Center partnerships to identify and close gaps in health care and to improve quality measures for Medi-Cal members. Since then, their business model and product have evolved to meet the needs of plans, providers, and Medi-Cal members and to leverage reimbursement pathways under CalAIM.
