The California Improvement Network has identified addressing social needs that impact health as a top priority area for 2018–19. This webinar explores promising strategies to address social needs that were implemented by four unique organizations: CBC Independent Practice Association, Kaiser Permanente Northwest, La Maestra Community Health Center, and SCAN Health Plan.
This webinar will cover each organization’s efforts, including successes and challenges, related to:
Implementing social needs assessments (both the national PRAPARE tool and “homegrown” assessments will be reviewed)
Implementing a community resource directory and referral system, with a focus on Healthify, Aunt Bertha, and “homegrown” systems
The staff champions that lead social needs screening and referral programs
Panelists from each of the spotlighted organizations will be available to respond to questions following the presentation.
