The California Improvement Network has identified addressing social needs that impact health as a top priority area for 2018–19. This webinar explored promising strategies to address social needs that were implemented by four unique organizations: CBC Independent Practice Association, Kaiser Permanente Northwest, La Maestra Community Health Center, and SCAN Health Plan.
This webinar covered each organization’s efforts, including successes and challenges related to:
- Implementing social needs assessments (both the national PRAPARE tool and “homegrown” assessments will be reviewed)
- Implementing a community resource directory and referral system, with a focus on Healthify, Aunt Bertha, and “homegrown” systems
- The staff champions that lead social needs screening and referral programs
