Eric McNair Scott is director of Community-Clinical Linkages at the Southwest Washington Accountable Community of Health (SWACH), where he oversees the regional HealthConnect Hub — a neutral nonprofit entity and connective infrastructure that helps communities organize and share information for community-based care coordination activities.

Prior to working for SWACH, Eric implemented health education and care coordination programs for high-risk patients at a Federally Qualified Health Center.

Eric has worked in systems change and equity-focused health and education projects for three decades, domestically and internationally. He holds a master’s degree in international and intercultural management from the School for International Training, with a concentration in Sustainable Development.
