About this grant

To support the potential future expansion of the Alameda County Care Alliance Advanced Illness Care Program to Los Angeles by engaging local stakeholders to determine interest and identify partners for a potential future pilot, and for a needs assessment to better understand the advanced illness care and caregiving needs of the local communities, to inform next steps.
