About this grant

To advance cross-sector collaboration in California to benefit Medi-Cal beneficiaries with complex medical and social needs by providing technical assistance on the Whole Person Care Pilot implementation, developing and sustaining a peer network among pilots to identify and share best practices, and informing state policymaking, reporting requirements, and program oversight.
