About this grant

To convene California public hospital system (PHS) leaders to share promising practices, troubleshoot challenges, and connect with their peers with a focus on improving their primary care delivery and population health management capabilities, and to develop strategies to strengthen relationships and facilitate collaboration between PHS and Medi-Cal managed care plans.
