About This Event

Accountable care organizations (ACOs) are growing in number and importance in the quest to improve care and reduce costs. The promise of ACOs lies in provider groups, hospitals, and others collaborating across the care continuum — in an environment of aligned financial incentives — to improve coordination, care delivery, and efficiency.

But there are significant barriers to making the promise of ACOs a reality. Representatives from Heritage Accountable Care Organization, a Medicare Pioneer ACO operating in Central and Southern California, will share their experiences over the last 18 months in relationships built across their affiliated medical groups and independent practice association (IPA), barriers encountered, and lessons learned. In particular, they will share experiences with beneficiaries, providers, and the community, collaborations with institutions, and success stories to date.
