CIN Partner Meeting — Addressing Social Needs That Affect Health

This partner meeting of the CIN aimed to continue to establish network connections and build relationships among partners with the goal of accelerating the spread of innovation in care delivery. As the only network in California that brings together commercial and safety-net provider organizations, health plans, and quality improvement groups, CIN fosters valuable cross-sector relationships across the state.
The meeting, which took place at the California Community Foundation, focused on the topic area of making improvements in supports for social needs that impact health, with an emphasis on controlling total cost of care for patient and member populations.
Speakers from Kaiser Permanente (KP) discussed the moral and business case for addressing social determinants of health and described a “social services resource locator” tool being developed and deployed across all KP regions. Additional partner organizations shared efforts to improve population health and manage costs by addressing social needs.
