What defines an exceptional patient experience? Immediate access to care? Personalized service? Compassionate providers? Spiritual and emotional support? A clean environment? All of the above?
CHCF, in partnership with ExperiaHealth, a nationally recognized patient experience advisory firm, is offering eight community clinics the opportunity to transform their patient experience through the Patient Experience Action Community (PEAC).

The goals of the two-year PEAC project are to:
- Devise and implement ongoing advancements in patient and staff satisfaction
- Create regional showcase sites to serve as demonstration models of exceptional patient and staff experience
- Develop and test standardized patient experience metrics
- Link improvements to financial, clinical, operational, and strategic outcomes
In February 2012, grants were awarded to:
- Anderson Valley Community Health Center
- Axis Community Health
- Golden Valley Health Center
- Lake County Tribal Health Consortium
- Neighborhood Health
- Santa Rosa Community Health Center
- Share Our Selves
- West County Health Center
Selected clinics will receive $40,000 for their participation; one-on-one coaching sessions with experts from ExperiaHealth; peer networking and access to national experts; and national recognition (successes will be showcased across California and the nation).
