About This Event

Inappropriate emergency department (ED) use — estimated to account for 35% to 40% of all visits to the ED — is often associated with limited access to primary care. North Vallejo Patient Access Partnership’s “Right Care, Right Place” program coordinates referrals between Sutter Solano Medical Center (SSMC, a nonprofit hospital) and La Clínica North Vallejo, a new clinic site colocated on the hospital’s campus and part of the federally qualified health center La Clínica de La Raza.

The program, which has been running for about three years, has seen a 41% reduction in ED follow-up visits at SSMC.
