Located near Boulder, Colorado, Clinica Family Health Services (formerly known as Clinica Campesina) has been transforming the way it provides care, with the goal of creating a true health home for its patients. The work has encompassed population management, EHR adoption, robust team care, and most recently, a sharper focus on improving the patient experience.
